3) After the form is submitted, it is not possible to change the phone number or address.
4) Take note: The information provided on this form is final and will be used for certification.
5) The IYO® International Yoga Organisation is unable to alter the certificates once the form has been submitted.
6) It is not feasible to change the certification's photo, name, address, or phone number.
7) In terms of certification, this form shall be considered final.
8) Before submitting, students must carefully review the form and ensure that all required fields are filled out.

How to Submit Your Details for Verification

To ensure a smooth and accurate verification process, please follow the steps below carefully. Provide all the required information in the specified format.

1. Attach Your Namaste Picture
- Take a picture of yourself standing in the "Namaste" pose, similar to the sample provided.
- Ensure the photo is clear and professional.

2. Submit One Identification Document
Attach a copy of **any one** of the following valid identification documents:
- PAN Card
- Aadhar Card
- Voter ID
- Driving License
Make sure the document is legible and up-to-date.

3. Provide Your Yoga Training Details
- Mention the name of the yoga school where you completed your training.
- Ensure the name matches the official records of IYS® - International Yoga School

4. Enter Your Correct Name
- Write your full name exactly as it appears on your PAN card, Aadhar card, or any other submitted document.

5. Provide Your Address
- Include your complete address, ensuring it matches the postal code (Pin code/Zip code) associated with your location.
